Procedure for drawing up and submitting requests for Public information

The provision of public information by Lutsk National Technical University is performed in response to the request on obtaining public information in accordance with the Law of Ukraine "On Access to Public Information".

 

 In order to simplify the procedure of making requests for information we suggest submitting a request by filling in the appropriate forms of requests for information.

 

The request for information must contain:

1)    name of the requestor, postal address or email address, and phone number if any);

2)    general description of information or type, name, requisites or content of the requested document (if these data are known to the requestor);

3)    signature and date (if the request is submitted in written form).

 The request for public information may be performed:

 Notes: 

1. Forms of requests for public information can be obtained in Lutsk National Technical University at the address: office No. 401Б, 75 Lvivska St., Lutsk.

2. The request must specify a method of obtaining information.

3. The answer to the request is provided in a manner selected by the requestor within five business days.

4. If the request relates to information necessary to protect life or liberty of a person, environment, food quality and consumer goods, accidents, natural hazards and other emergencies that have occurred or may occur and threaten safety of citizens, the answer to this request must be submitted no later than 48 hours after receiving request.

If the request concerns the provision of a large volume of information or requires information search among a significant amount of data, the period of the request consideration may be extended to 20 working days with justification for the extension. The requestor is notified on the extension in writing not later than within five working days from the date of its receipt.

6. Information on request is provided free of charge.

7. If the requested information includes documents of over 10 pages, the requestor is informed with specifying the amount of the actual costs on copying or printing documents, and details and procedure of reimbursement of such expenses within five working days from the date of the receipt. Information is provided within three working days after confirmation of payment for the actual costs.

8. A request may be refused in the following cases:

8.1. Lutsk NTU does not have and is not obligated (according to its competence prescribed by law) to have information on the request;

 8.2. The requested Information belongs to the category of information with limited access;

8.3. The requestor has not paid the actual costs on copying or printing of documents in accordance with paragraph 7 of these notes;

missed the requirements

8.4. The requirements for preparation and submission of the request for information stipulated by part five of article 19 of the Law of Ukraine "On Access to Public Information" are not complied. Namely, the following data are not specified:

surname, name, patronymic (name) of the requestor, postal address or email, telephone number (if any);

general description of information or type, name, requisites or content of the requested document (if these data are known to the requestor);

signature and date (subject to submission of a written request).
